Providing one-on-one support for an individual to participate in vocational discovery, identification of career interests, and development of a plan to achieve community integrated employment or self-employment goals. The outcomes of this service are an identified career path and the supports needed for pursuing that career path, determination of the individual's stated career plan objective(s) which specifies the individual's needs, interests, strengths, natural supports, and characteristics of potential work environments, along with training or skills development necessary to achieve the individual's career goals. A completed career plan/discovery profile is used to guide ongoing individual employment support needs.
If an individual is currently employed, this service may be used to explore other ongoing employment career objectives which are consistent with the person's skills and interests or to explore advancement opportunities in their chosen career goals. Transportation is included during the time in which this service is provided.
Career planning activities may include but are not limited to the coordination and delivery of:
